If someone wants to submit the form to us, that is fine. (It would be helpful if he or she would include the SSN and/or RIN of the individual seeking services.) But please note: 1. This form is not necessary for individuals in the community who are enrolled in PUNS. If they are currently active on the PUNS database, and they meet the Ligas criteria, we have included them in the class member listing. 2. For persons who are residing in ICFs/DD, this form is one of three documents the Parties have agreed to accept as affirmative statements that the individual wants to move from the ICF/DD to community services. [The other two forms are the DDPAS 10 Form and Waiver Form IL-1238. Both available on our website.] More information about our outreach to potential class members will be available in the draft implementation plan. The draft plan was shared with the Plaintiffs and Interveners on 9/15/11. They submitted their initial comments, as requested, by 10/3/11. We are now working to incorporate those comments as appropriate. It is our hope a revised draft will be made available for comment by organizations such as yours on or by 10/24/11.
