Call the office. Since you are a client, staff will email to you, or mail to

you if you prefer, the appropriate form as a courtesy. There is an " adult "

version of the Short Term Guardian Declaration, wherein you are delegating not

your parent authority, but now your Guardian of an Adult Disabled Person

authority. It can be used for up to 60 days in a 12 month period. We were able

to get that law extended to Adult Disabled individuals 11 years ago, when my

son, Mitch was 19. We started when he was turning 18 and it took us a year to

get through the house and senate in Springfield.

Does anyone know what we may need to do before leaving our daughter with her

grandparents when we go on a short vacation? In the past we have used a vacation

power of attorney that we got from when he did our Special Needs

Will. Our daughter will be nineteen and we are her legal guardians. Can we still

use the form we did when she was younger, or is there something new we should

use, or do we need anything at all? We will be taking her to Missouri and flying

out from there so I know everyone would feel better if there were no hang ups if

anything medical came up.

Thanks , I just called your office and they're getting it out to

me. It's good to know that if anything comes up while we're on a much

needed break that her Grandparents will be legally able to handle it.

Beware, the Adult version is ONLY if you are Court appointed Guardian, and

if Limited Guardian it only delegates the authority stated in the Court

order. The special needs attorney who you work with should provide to you,

for no additional cost, both the Short Term and Stand-By Declaration forms

as a well as help in their completion. Remember, both forms require

witnesses and notary.
