Employment Survey

This scorecard was created to help state teams review the state policies,

practices and strategies that impact on opportunities for integrated employment.

The answers on this scorecard will help state teams identify priorities and set

goals for employment initiatives and outline a strategy for doubling their

state's rate of integrated employment by the year 2015. Please rate your state's

level of agreement with each statement below by selecting the number that best

reflects your state's current situation. Your comments are welcome.

Please complete prior to May 15th. At that time, we will run a report and begin

discussions on a state action plan.
