Do not be in a hurry to fill it out. Can your PAS agency help? The first line says " We have to checkif you can still get medical benefits. We need your help " Then goes on to say that the form needs to be filled out & sent back in. Also thatthey need copies of proof of: your family's income for the last 30 days (except for Social Security & SSI), the amt of your family's checking & savings accts. anything else we ask you to send us in the questions on this form. Has to be back by Jun 10th, though I can call the caseworker before that date & tell them I need more time; or at least , since the letter is adressed to him. Hopefully they know who his caseworker at the office is, cause I don't. We don't deal with one. started getting SSDI in Feb. & doesn't have other income. I'd like you to take a look at the letter. Everyone gets a form for an update: to review to see if the person is still eligible for Medicaid. It usually has a 10 day deadline so be careful that you timely complete & return this to the Medicaid office. If your child is over 18, he is considered his own household so only his income & assets should be put on the form. This form and all requested documentation (proof of health insurance, proof of income, proof of assets...etc...) must be timely returned or the case can be cancelled. Some offices will send a second request before cancelling a case.
