I was told by a friend that there is a form you can complete after one year that gives you pernament guardianship and you don't have to go back to court. Is this true? Anyone know what the form is? I live in DuPage co. (Naperville) Also, I have some PSW hours I need to fill. If anyone knows someone who is looking for extra hours feel free to give them my email. If you get the report in before the deadline, you won't have to appear in court. They usually send it 2-3 months before the anniversary date of when you were granted guardianship. > > We currently have guardianship of Zackary. Also, not all Counties require the annual report. Further, some counties require it not every year, but rather every two, or every three years.
