 We are conducting a study looking at end-of-life decision making

for people with an intellectual and developmental disability from the

perspectives of clients, staff and family. At this point, we are hoping to

recruit family members who have experienced the loss of someone over age 18

with an intellectual and developmental disability. I have attached the

family recruitment material and consent form in order to give you a better
