Signing the Ligas document let's the court and the state know that your loved

one is on the PUNS, requesting and in need of community based supports and

services. The individual may already be in an ICF-DD and wish to move to

community OR be living at home, awaiting community services. In either case, it

is imperative that you fill out the form and return it as requested, to be

included in the list of eligible individuals. This could be an opportunity for

many...The agreement would give more housing choices to 6,000 adults who receive

state aid and live in privately operated care facilities (ICF-DD's). Illinois'

Medicaid program also would be required to offer services to an additional 3,000

be on the PUNS list to get anything in the future.

If one is on the PUNS but has NOT received the letter, can you still go to the

website and find the form and fill it out?

Look on the Equip for Equality website www.equipforequality.org. The info is

right on the homepage.

This is so incredibly important. This is your chance to get a services in the

next 6 five years. But, you have to make sure that your family member is on

record as wanting services. The details are on Equips website.

Just so you know, F.T. and other...attachments do not come through on

IPADDU postings. However, if you send me an attachment, I can put it in the

IPADDU files, which is accessible to everyone. Better yet, if it is short

enough, you can also cut and paste it into the body of a posting.
