Greetings from Kent State University - Kent, Ohio:

We are conducting a national survey on parent/caregiver needs and priorities

in planning for their adolescent or adult child with intellectual and

developmental disabilities.

If you are a parent or other caregiver of an adolescent (16 years and up) or

adult with a disability, you are invited to participate in this research study.

If you know someone who meets this description, we would appreciate it if you

would forward this information to them. The research is being done to better

understand the opinions and experiences and of families who are currently

providing care for family members who are adolescents and adults with

disabilities.

To participate in the study, participants are asked to complete an online

questionnaire called the “Parent Perspectives on Planning Future Residential

Options for Adolescents and Adults with Disabilitiesâ€. The questionnaire

includes 29 questions and should take approximately 15 minutes to complete. The

questionnaire is anonymous-you will not include your name or other identifying

information and your identification will not be tracked online with the survey

software.

Taking part in this research study is entirely up to you. You may choose to

participate or not and if you wish, you may discontinue your participation in

the questionnaire at any time without penalty. Completion of this questionnaire

indicates your consent to participate in this research. This survey has been

reviewed by the Kent State University Institutional Review Board for Human

Subjects Protection and meets the guidelines for Human Subjects for Level One
