CHOICES ­ Tennessee Long Term Care Services Program

SUMMARY: In June 2008, Tennessee passed legislation restructuring

LTC service delivery, including the expansion of HCBS services by

integrating LTC within a managed care environment and providing

consumer direction options. In March 2010, the state implemented the

CHOICES program, wherein persons enrolled in TennCare that needed LTC

services would receive them through a managed care organization.

Supports and Services

SUMMARY: Five innovative states (Arizona, Hawaii, Tennessee, Texas,

and Wisconsin) with demonstrated expertise in managed care

SUMMARY: The information and assistance (I & A) service is a

cornerstone of the ADRC. This guide provides tools that I & A

supervisors can use to cultivate, mentor and maintain a top-notch

team of professionals. It incorporates tools developed and used by

two ADRC's in Wisconsin, and includes tools for recruitment and

hiring, ongoing communication, training, and performance feedback

and appraisal. A number of sample forms are provided.

Technologies for Improving Post-Acute Care Transitions

SUMMARY: People receiving long term supports and services are

frequently at risk for repeated hospital admissions, and patient

care after a hospital stay is often fragmented and uncoordinated.

Several types of technologies that have potential to support

post-acute care transitions are discussed in this position paper.

Studies suggest that the use of such technologies can lead to fewer

hospitalizations and emergency room visits, high patient

satisfaction and acceptance, and reductions in cost of care.
