Integrated Care Program Save the Dates

Save the dates

The stakeholder meetings for the Integrated Care Program will be continued into

2011. The meeting dates will be: January 13, 2011, January 20, 2011, and

January 27, 2011. Information on the topics to be discussed will be sent out

prior to the meeting on January 13, 2011.

All meetings will be held via videoconference, from 1-3 p.m., at two locations:

401 S. Clinton, 7th Floor, in Chicago, and the Bloom Building, 201 S. Grand, 3rd

Floor, in Springfield. No dial-in option is available for the meetings.

As follow-up to the December 16, 2010, meeting, attached is information on each

plan's care coordinator caseload thresholds.
