When should I start medication

I just stared this journey in march after getting a bad case of the shingles

then I was tested for hiv and thats how I found out .my t-cell count is at 425

and my viral count is 300,000.So they have not started any medictcations yet and

I thought if I already had a illness due to the hiv I should start ?
