Here is the application which I will also put in the files section: Notice of Mini Grants Available Micro Industries for Self-Advocates Offered by Life's Plan Life's Plan is a non-profit organization that provides Special Needs Trust options for people with disabilities and their families. Life's Plan is a Medicaid qualified trust providing supplemental care and services through our personalized trustee services and pooled investment strategy. Our individual pooled trust accounts do not jeopardize key public benefit programs such as SSI and Medicaid. The families we serve have an alternate choice to spending down their loved one's inheritance with the use of a pooled trust. Life's Plan has been serving individuals with disabilities and their families throughout Illinois since 1986. Who is Eligible: Adults with Developmental Disability living in Illinois Limits: No more than one individual or one partnership per social service agency will be selected. A total of ten (10) awards will be made up to $2000 per person or partnership. Focus: To fund start-up money for micro-industry business that allows adults to earn wages. A micro-industry is a business run by a person with a disability with support from staff to allow the person to be more economically independent. Examples of these types of micro-industries include but are not limited to: a shredding company, jewelry making, used book resale, greeting cards, etc…. Requirements: Proposals should be limited to no more than four (4) pages. Proposals should include: 1.Basic demographics of person applying 2.Summary of the micro-industry purposed 3.Projected outcomes from the business 4.Budget on how the funds will be spent 5.A one (1) page work plan with time frames 6.Indication of who will support you in this endeavor 7.Willingness to submit a 1 page final report by 12.31.10 8.Indicate to whom or to what entity the funds will be dispensed so that the individual's government benefits are not compromised. Application can be e-mailed to: snixon@... or mailed to Nixon, Life's Plan, Inc., 2801 Finley Road, Downers grove IL, 60515. Proposals will be accepted until 5:00 PM, February 15, 2010.
