Figure 3illustrates this regulatory step in the oxidation/organification of iodine.As previously mentioned, the oxidation of iodide to iodine occursthrough the interaction of H2O2 and TPO. Iodine is a necessary product inorder to provide the correct molecule in the cell so that organificationcan occur. If organification does not take place or is blocked, thyroidhormone and iodo-lipids will not be formed. As can be seen from Figure 3,this reaction is controlled by intracellular calcium levels and iodinatedlipids-iodolactone.Intracellular calcium stimulates this pathway. On the other hand,-iodolactone and other iodinated lipids act as a brake on the system. Ifthere is not enough iodine in the cell to organify and produce adequateamounts of -iodolactone, it can set the stage for damage to the thyroidcell and the development of an autoimmune thyroid disorder such asHashimoto's or Graves' disease.A Proposed Mechanism for the Development ofAutoimmune Thyroid DisordersThe NADPH oxydase system is found in the mitochondria of our cells. Themitochondria are the energy-producing cells of our body. The mitochondriaproduce energy (i.e., ATP) through a complex process called oxidativephosphorylation. All medical students (and most physicians) are familiarwith oxidative phosphorylation because we have to memorize the many stepsresponsible for producing ATP. This production of ATP requires many itemsincluding: oxygen, magnesium, ADP, and amino acids.Many people with chronic illnesses, such as fibromyalgia, chronic fatiguesyndrome, and autoimmune disorders, complain they have no energy. ATP isthe molecule that stores energy for the body. The body is constantlyproducing and utilizing ATP. Its production is a complex process that isbeyond this book. However, there are two cofactors, Vitamins B2(riboflavin) and B3 (niacin), that are integral to stimulating oxidativephosphorylation and ATP production.Hydrogen peroxide is a byproduct of oxidative phosphorylation. It is thisproduction of hydrogen peroxide that is so critical to the oxidationprocess of iodine. Hydrogen peroxide and TPO help to oxidize iodide toform iodine.If there is a deficiency in iodine, which is common when ingesting the RDAfor iodine, there will not be enough substrate (i.e., iodine) to produceiodinated lipids. As can be seen from Figure 3, the lack of -iodolactoneand other iodinated lipids results in a loss of the 'brake' in the pathwayto oxidize iodide. This may result in a temporarily production of too muchhydrogen peroxide. This excess hydrogen peroxide can damage the enzyme TPO.What Happens If TPO Is Damaged? Autoimmune Thyroid IllnessThe body's response to TPO damage is to produce antibodies against TPO oranti-TPO antibodies. A diagnosis of Hashimoto's disease requires thepresence of anti-TPO antibodies. As the damage worsens, surroundingproteins can also be damaged such as thyroglobulin. Damaged thyroglobulinwill result in the body producing antibodies againstthyroglobulinanti-thyroglobulin antibodies.In most cases of Hashimoto's disease, there are antibodies to both TPO andthyroglobulin present. Although Graves' disease may also possess thesesame antibodies, antibody production is not necessary to make the diagnosisof Graves' disease. However, my clinical experience has shown that thetreatment for both Hashimoto's and Graves' disease can follow a similarcourse with similar positive outcomes.How to Treat Autoimmune Thyroid Disorders1. Ingest enough iodine in order to provide adequate substrate toiodinate lipids.2. Take Vitamins B2 (Riboflavin) and B3 (Niacin) in amountsnecessary to stimulate the NADPH system to produce adequate amounts of H2O2.3. Correct oxidant stress in the thyroid gland and the mitochondria withantioxidants.4. Ensure adequate magnesium levels.5. Minimize oxidative stress in the body.14. My TSH went up on Iodine.This is a common occurrence when a person starts to take iodine after beingdeficient. The body increases TSH to stimulate production of thyroglobulinwhich is used to bind to the iodine in thyroid hormoneproduction. Individuals can have TSH levels as high as 75 for up to 6months without clinical signs of hypothyroidism. The best values to checkwith labs if there is a concern are the Free T3 and Free T4. You will findin most cases that the "frees" are nicely within the ranges.15. I have adrenal fatigue. Can I take Iodine?Yes, Iodine is used by every gland and mucosal lining and is needed to healthe adrenals. You should also use unrefined salt to support them as wellas Vitamin C. Other products such as ashwaganda, licorice root, adrenalglandulars and in severe conditions a doctor can prescribe hydrocortisoneto replace the body's deficiency.16. This isusually met with jubilation that they are at a good level. But wait! Thisisn't a place to stop. A bromide level test has become available and canbe added to the loading test. This is highly recommended because it givesa clearer picture of what the TRUE status of the saturation level is. As ageneral rule if you see a higher saturation level (80% or above) with abromide level over 10 mgs/L (the upper limit of normal per Dr. Guy Abraham)you are bromide toxic and the receptors that would normally be able to pulliodine into the cells cannot because bromide is blocking thereceptors. This will result in a high level of iodine being eliminated inthe urine indicating (falsely) that you are more saturated than youare. Persons with results such as these should consider supplementing withLugol's or Iodoral in doses of 50 mgs or more for 6 mos to 1 year and thenretest Iodine Loading and Bromide levels to see what the new statusis. Dr. Abraham has stated that this amount of time is needed to makesignificant progress in detoxing and to build saturation in thecells. After supplementing for 6 mos to 1 year, the trend has been to seesaturation levels go down while bromide elimination levels increase.Another reason for a false high reading in saturation levels is due todefects in the symporters of the cells. B2 and B3 are what is in the ATP CoFactors supplement that is recommended to help you use the iodine you take. I have just started taking that in addition to a B Complex. I had a test that showed very poor iodine uptake and hypothyroid. Those 2 vitamins are exactly the ones needed to help that. So no, I wouldn't cut back on B2 and B3. If you have it, Dr. Brownstein's Iodine book, p. 166 explains how these help some people. Guest guest Posted July 7, 2012 Report Share Posted July 7, 2012 I am 20 years post open-chest HM. I had about 85% relief that lasted at least 5 years. I was very pleased with this result considering that I had difficulty swallowing liquids prior to the surgery. Over the subsequent years I had several dilations and recently a Botox injection to " touch up " later swallowing problems. Sorry to report that the surgery appeared to have limited results on reducing very painful esophageal spasms. More recently, age and quitting drinking alcohol has reduced, but not eliminated, the most painful spasm attacks. I can often stave off the pain by drinking copious amounts of cold Diet Coke and taking Levsin under the tongue. > > Hello All, > > I haven't posted in a while.
