Guest guest
Dear Forum members, This is regarding the message by Dr Sajithkumar and the reply commends from Dr Bitra : I do share the concerns by Dr Bitra and yet to prescribe a 2 drug regimen. But when we see patients dying in front of you and if you are sure that you can prolong their life and reduce their sufferings with 2 drugs at least for few years. Is it ethical to withhold it? The main worry is about resistance. One of my senior colleague prescribe it to pts whom he think are unlikely to transmit resistant strain. As the prices are falling we are going to see lot of misuse and resistance esp. from developing countries. But fortunately there are a no. of newer drugs. Regarding I really doubt if we should get a viral load assay to monitor our pts as clinical response is that what we are aiming at. I am not saying that it should not be done but inability to do these tests should not prevent the pt getting the advantage of HAART. I do agree that the need of the hour is to check the NACO recommended guidelines and initiate discussion on its practical applicability.
