verification slip to visit farm

I sent this earlier but didn't see it...Outlok Express is acting up today, I

think.

My son's preschool is going to another student's farm. This is

what I had to sign...

" Dear Teachers and Parents,

Your students/children are planning to visit our farm in May. Due to the

outbreak of Foot and Mouth disease in Europe, we will need to take some

extra precautions in order to protect our herd.

Please have all parents sign the following form stating that their child has

not been out of the country or around anyone who has been out of the country

during the four weeks prior to the tour date.

We love to have tours at our farm, but we also need to protect our

livelihood from a potentially catastrophic disease.
