Vaccine Registry - Birth Certificate

There is a box on birth certificates that asks if you want your new baby on the vaccine registry. Unless you paid careful attention when you filled out the birth certificate, and especially if you gave birth in a hospital, the box probably was checked yes. Most hopitals automatically check the box for you and lots of people don't realize all the information asked for, especially since someone is usually standing over you acting impatient waiting for you to sign.

This was told to me on another list, can anyone remember seeing this on the birth certificate application??
