On the Illinois Guardianship and Advocacy Commission website are Legal Forms you can use: HYPERLINK " http://gac.state.il.us/forms.html " http://gac.state.il.us/forms.html . Go to Forms for a “Guardian of a Person” to Use. There are two types, choose what best fits you. At the end of the page are Probate Forms for a few other counties in IL. Since you can download the form it makes it easy to type in what information you want to submit. We have also used the Short Term Guardian form when we are away on vacation. I have heard some counties send the form to the guardian to complete but in the years we have been guardian for our son we have never been contacted by the court in Cook Co. I once asked a lawyer who was presenting at an ARC conference and she said that the presiding judge does look over the papers as they come in.
