Again and again one needs to understand and explain that I do not disagree with the importance of discussing the matter with the patient, making an informed decision, importance of nutrition, giving up substance abuse, positive frame of mind, good social support etc. etc. etc. The issue is that these are not enough to save lives. If someone takes an informed decision to rely only on these I would also support the decision. But if the drugs are seen to help those who are doomed to a life with extreme morbidity and at times an early mortality then despite all their limitations I think they are certainly worth giving a try. I do not support hit early hit hard, but they work better and probably have fewer side effects if started when CD4 are around 300-250. Unfortunately most patients get diagnosed when they are in a worse shape. My only worry is people relying only on alternative treatment methodologies should not wait for too long before deciding to start ART as a last resort. My only contention is that at a time when we are seeing a ray of hope let's not try and extinguish the same by putting in arguments which are more emotional and less rational. Otherwise we all know anything may be claimed to be treated and cured just by faith. Once again I am deeply concerned about those who are unfortunately suffering from the side effects but I am equally, if not more, worried and concerned about those millions who do not have anybody to discuss whatever options you or me are suggesting. Out of these so many could survive with all the support you suggest and THE MEDICINES, until the CURE. And here I am not suggesting that Drugs are going to be the panacea for them. All GPs should be properly trained in the prescription of these meds and their toxic effects.
