pulmicort respules questions

Hi everyone,

I am about to start the pulmicort 0.5mg respules mixed with my neilmed

rinse at the urging of my ENT who just did my 3rd surgery. I am

concerned about the dose (doing this twice daily). Has anyone else

done this and what type of results have you had? Any tips? Thanks, Dawn

This has worked great for me with DISTILLED water.

See post #12896 about the dose. The notice says it is very probably safe but

that side

effects can't be totally excluded. Check regularly (ie at least twice a day) if

you have any

modification of vision like slight blurring. I know it may not be easy to

distinguish it from

eye fatigue, but if you experience something that clearly does not feel and

behave like

usual eye fatigue, it may mean the optical nerve is getting too much corticoids

delivered

too close to it. In that case, ask your doctor for advice, but you could stop

until the

blurring or whatever disappears, and then resume at half or a quarter of the

initial dose,

and see if it happens again. Also, the dose absorbed by the mucosa depends on

the time

the irrigation liquid stays in contact with it ; if you kept the rinse liquid

inside 1/2 minute,

say, your mucosa would absorb more pulmicort than during a 5-second rinse. If

your ENT

prescribed a long-term irrigation routine, you could take the first few weeks as

a trial

period to find the smallest dose that achieves the desired effect on the long

run.

What do you mean you are concerned about the dose? I'm not sure what

that means.

I am doing it and I am seeing no effect so far. :( I'm not putting it

in the rinse, though, I'm just laying back and sort of squirting the

liquid directly out of the respule into my nose. It's too much liquid

as it is, I certainly don't want to dilute it further.

P.S. since these respules are for children it's hard for me to

believe the dose would be too much. I would rather believe it might

not be enough (which certainly seems to be the case for me).

Just wondering where you have the information that more steroids are

absorbed the longer it's in contact with the mucosa -- I mean, this

theoretically makes sense, but has this been studied? I mean, this

formulation isn't even intended for direct contact with the mucosa as

a liquid as far as I know, it's intended to be nebulized.

In fact, I meant that if you irrigate with corticoids or whatever

else diluted in saline, like I do sometimes, the more you keep the

solution inside the nose, the more molecules get into contact with the

mucosa, so the higher the absorbed dose from the point of view of the

mucosa.

Besides, it makes sense that a minimal time of contact - I don't know

precisely how long, seconds, tens of seconds? - would be required for

good tissue penetration and saturation of all receptors.

If you irrigate with undiluted corticoids, watch out for any eventual

vision symptoms ; I had slight unilateral vision blurring after

nebulizing for 1 1/2 day with 4mg dexmethasone twice a day, which is

what my doctor prescribed, and that is a clear warning of impending

danger for the optical nerve.

There are cases in the literature of people who got blind from too

much corticoids. So I immediately washed with saline, stopped

nebulizing until the symptoms disappeared, and resumed with a quarter

of the dose - it worked about as well without any symptoms. But I

still wouldn't do it on the long term even at that lower dose.

The Pulmicort IS in a 2 mL solution already, so it is diluted.

It makes logical sense that a minimal time of contact would be

required, but that is not really what you were saying -- you were

saying that the longer the contact, the more absorption. Honestly

how do we know that the Pulmicort is being absorbed AT ALL? It's not

formulated for use up the nose and hasn't been studied for this.

What do you mean " nebulizing " with dexamethasone? Was that for your

asthma and you nebulized the dex into the lungs? Thanks.

By nebulizing with dexmethasone, I meant breaking a dex ampule into a

compressed-air nebulizer and inhaling. Most of it goes into the lungs

(I have no asthma), but some goes in the sinuses if the nebulizer

delivers airborne particles of the right size (the notice should say

if it is a lung nebulizer or a sinus nebulizer ; some have a frequency

switch for that). Since I have no asthma, I do it only on

prescription, to calm the sinuses so that antibiotics may reach in.
