What causes such drastic changes in appearance?

Hi All,

When looking at all the photo's of before and after I would like to know if it

is only swelling and puffiness that makes you look so different. The changes are

so bad, I used to not be able to recognise myself in the mirror, could this

just be down to the mucin that is in all the cells? Does anyone know the answer?
