I am especially thankful to Dr. Surendra, Amit Panchal for their important contribution and also all NetRUM members who took efforts to read all the posts of the discussion. Various medicines are under investigation using nasal/pulmonary routes of administration. These include small molecules, protein/ peptides, and genes, etc. Pulmonary/ nasal medicine delivery offers great potential for both local targeting for the treatment of respiratory diseases and systemic targeting for the treatment of diabetes mellitus or thrombosis, tuberculosis, epilepsy, etc and also for effective vaccination. Patients planning to use such delivery methods, practical issues, such as the time needed for medicine delivery and the size of the device will be most important to them. Cost is also an important parameter since newer devices appear to cost more than the previous ones. However, equally important parameters as cost are compliance with treatment and quality of life. The less time these, mainly young, patients spend using a nebulizer along with the ability to carry their medicine delivery system easily makes their life as close to normal as possible. This may counterbalance the increased cost paid for their treatment. But this is not the case with poor patients in the developing economies. We recognize the importance of quality of life, but the cost of the delivery system must not be an obstacle to the treatment of patients, especially if clinical effectiveness does not differ.
