Unless we have evidence it is like shooting in the air (blind treatment). Patient may improve or may not, it is at the discretion of the treating physician. In certain settings, clinical experience or judgement of the physician may be quite beneficial for the patients but we need to have evidence before treating any patient. Otherwise, it may end up with increase healthcare cost and more frequent hospitalizations or further complications. Who has to generate evidences? How to believe evidences? Which evidence to follow? How evidences are related to pharmacoeconomics? so many questions arise when we discuss this important and sensitive issue. Let us talk about these issues with certain evidences.
