need help with disability codes

i was turned down for Medicare from the state cause i and my

wife made to much money. but on the other hand the case worker

said that i was approved on the disability does anybody know

what the code number 3490 means? and what the voc rule number

202-02 is? i,ve not heard from SSA at all just from social

services the letter from dds is date 7-22-03 received at social

services on 8-07-03 now what? does the SSA get the same form or

is it a different form?
