At the time of selecting this topic for discussion I had gone through the list of 100 discussions held over Netrum (published in March issue of our monthly newsletter). Though I could identify a couple of discussions held on Pharmacovigilance in general, there was none on enhancing ADR reporting in particular, hence this discussion. But of course, it is quite possible that during those two discussions members could have touched upon this aspect too. Anyways I am not sure whether hospitals of our country have a fool-proof mechanism of reviewing and ascertaining the cause of patient deaths occuring in the hospital. How helpful could monthly or weekly review meeting be in identifying adverse events occuring in the hospital and attributing causality and level of severity to them. These meetings could be held under the auspices of Drugs and Therapeutic Commmitte also. If every fatality in the hospital is probed thoroughly during such review meetings, we may be able to collect important information on the incidence of severe and fatal ADRs that go unreported. Furthermore regular periodic review committee meetings could be helpful in identifying less severe and non-fatal ADRs too. Kindly throw some light on this if possible. With Regards Dr. Geer M. However, to sustain this activity Each hospital should have a centrally place pharmacovigilance cell providing special services on ADR reporting and drug information centre. Arrange frequent meetings with clinicians and postgraduate students of different department specially medicine, psychiatry, dermatology Organize workshops and training courses on Ph vigilance for postgraduates on regular basis Involve pharmacist and nurses in the programme Integrate Pharmacovigilance as a part of UG and PG curriuclum including medicine, psychiatry, dematology etc. all the wards should be eqiped with a box Dr. Mira Desai M. The idea is to improve the system, educate the health care workers and not to victimizes anyone. The deliberations will bring out sensitive issues and definately help to find out the cause of death, provided all are taken into confidence and have a open mind set and support of higher authorities. I worked at university teaching hospital, Lusaka , wherein these meetings were held monthly on regular basis. Although there were hot discussions but it was a good learning for all of us. It also inculcated the practice of documentation. The observations/ conclusions should be used to inform and educate and not to victimize.regards,MiraDr. Mira Desai M.
