Hi docs,

I remember a time in the dark ages shortly after I graduated and was licensed, (boy it was hard to hang that stone tablet on the cave wall!) that a member of my class was disciplined by the OBCE for putting "Chiropractic Medicine" on his letterhead. Has that changed? I thought the practice of medicine involved the dispensing or prescribing of substances called medicines. What kind of bottle does one use to contain chiropractic medicine? I think chiropractic physician fits what D.D. was seeking but chiropractic medicine (even with a spoonful of sugar) is a little hard to swallow.

Welcome. Thank you for your insight. I look forward to future insights. For the most part my experience MD's are positive. As with everyone, they criticize most what they don't understand. Most MD's don't understand what we do. For that matter there are a lot of medical specialties and procedures that I am completely ignorant about. I applaud those that take a look at both sides of the fence with out self interest.

Interestingly, and to my surprise, none of the other chiros I knew in Chiropractic school went to medical school because they hated it either. In fact, all of the chiros in medical school I knew did very well (especially if they were from LACC/National/NYCC), were student government leaders, and were well respected for their previous professional degree. At no time was chiropractic picked on, preached against, etc. Instead, some of my professors sought out my opinion and care in regards to their aches and pains. Many others were clueless about chiropractic but willing to learn.

As a DC/MD in Oregon, I hope to be an advocate for the chiropractic profession in supporting evidence-based responsible chiropractic medicine by those professionals who are really trained to provide it.

Anyways, I hated to be a eavesdropper, so I thought I would introduce myself. Thank you for having me!

Interesting point Rick. At my last reading of the Oregon statutes and OBCE policy we are still required to place the words " chiropractor " or " chiropractic physician " under our names on any office signage and to do so in a certain size font as compared to our names. As far as I know, the term " Chiropractic Medicine " is yet not recognized by the OBCE.

But clearly this issue pertains more to our identity that just our signage. I present the following for your consideration:

I. BASED ON A DEFINITION...

According to dictionary.com the #1 definition of " medicine " is as follows:

1a. The science of diagnosing, treating, or preventing disease and other damage to the body or mind.

1b. The branch of this science encompassing treatment by drugs, diet, exercise, and other nonsurgical means.

I would purport that chiros most definitely fulfill (or should fulfill) definition 1a and are a branch of the medical sciences by definition 2a.

All too often, and by traditional means, much of today's society oddly refers only to " drugs " or " pharmaceutical drugs " as medicine. However, dictionary.com adds the following definition (emphasis and bold text added):

3. " An agent, such as a drug (BUT NOT SOLELY A DRUG), used to treat disease or injury. "

Again, I would purport that manipulative therapy, physiological therapeutics, lifestyle modification, and nutritional advice fulfill that aspect of the definition of " medicine " .

Finally, is it not often stated that we practice an alternative or complementary MEDICINE?

II. ITS NOT ALL OUR FAULT...

Yes, even an LACC/SCUHS grad can give BJ some credit. In my opinion, if it weren't for BJ and his fanaticism (maybe passion is a better word) the spark of the chiropractic profession would have been extinguished long ago. However, to save ourselves the profession voluntarily excused (bowed out) itself as a branch of medicine. In many States we even demanded to have our own board of examiners (I'm not sure of the history of the Oregon Board specifically) instead of becoming part of the OBME by which even acupuncturists are overseen.

But our incomplete title is not ALL our fault. The allopathic medical profession was the main show in town (ie. the world) for so long that even the " medical " degree is the Doctor of Medicine (MD, Medicinae Doctoris in Latin). The title infers and has encompassed the broadest scope of training and practice available which, for the most part, is still available to MD's today (if they choose to accept it). Back in the days when the chiropractic profession was maturing, the AMA based on fear and a poor understanding of what I am discussing here, would have freaked out if we tried to use the title " Doctor of Chiropractic Medicine " . This still may be true, but to a lesser extent, today.

Because of the bad feelings between " medicine " and " chiropractic " at that time I'm not even sure the chiropractic profession would have had any desire to use that title anyways -- BUT I would suggest that we might lean in that direction now.

Either way -- outside pressure or lack of desire-- our title has been left artificially incomplete. Doctor of Chiropractic WHAT?

III. What the heck is a DO anyways?

Many, mostly non chiros, ask me why I didn't decide to continue on to DO school since they also perform manipulation. Since that is outside the scope of my soapbox post at this time, I won't address that much. But it brings up an interesting concept. Has anybody noticed that Doctors of Osteopathy have been popping up all over? Has anybody asked a DO about their education? From most recent DO grads, you'll hear " I went to medical school at school XYZ " . Did you catch that? Most of us would.....and its not entirely forthright.

In my medical school clerkships I have been on the wards with an occasional DO resident or attending. I will be assigned some of his patients to take care of and the DO will stop by to discuss the patient with me and co-sign my notes and orders. They will also quite frequently stop in to meet and examine the patient to assure them they are being looked after by a physician and not just a student (and to assure things can be billed properly). Well it has not been infrequent that patients will ask me " When do I get to see a doctor? " I'll explain that I am a medical student and will be working closely with him or her but THEIR doctor should be coming in frequently to see them. The patients often say " No, I haven't seen a doctor since I've been here. " and continue " Some guy with a DO at the end of his name comes in every so often though. What the heck is a DO anyways? "

Most DO's have heard this question over and over again and are sick of it. They have realized that the title " Doctor of Osteopathy " wasn't working and was in fact incomplete. It didn't explain what they actually did. SO, the osteopathic profession did something dramatic - they added the word MEDICINE at the end of their name. Now they are called " Doctor of Osteopathic Medicine " (DO title remains), they take boards called the " COMLEX " =College of Osteopathic MEDICAL Licensing Exams, and their schools are called " Colleges of Osteopathic MEDICINE " . There has been a dramatic change in their level of respect by the general public and even the " medical " profession itself. They now attend medical school, take medical boards, and have a medical degree.

I'm not suggesting that the chiropractic profession should consider changing its name SOLELY for respect that comes inherent with the word " medicine " , but also because our current title is incomplete and not well understood by the general public. It would also signify an " extended hand " of professionalism to our medical colleagues (MD and DO).

I'm also not suggesting that we follow the ENTIRE example of the osteopathic profession in this regard. Even after the change we should NOT state that we went to medical school, that we took medical boards, or that we practice medicine. While those statements wouldn't be untrue, they are deceptive. Instead we could consider adding chiropractic to each of those.

IV. Our identity....Our Future

I'm not suggesting that what we call our profession will solely dictate our identity. Nor am I suggesting that a title change will fix the identity and utilization problems the profession now faces. Finally, I'm not stating that I think we should definitely make this change at all. However, I do feel we should discuss the possible benefits and downfalls among ourselves (on and off of this forum).

Despite the strong education, broad knowledge base, and great skills that chiropractic physicians posses, our image is that of DO-ERS and not THINK-ERS. In my experience patients come to us to get adjusted rather than to figure out what their problem is and THEN have it treated. By the influence of insurance companies and ignorance of the general public we have allowed ourselves to be pushed into the role of therapists (DO-ERS). We are not paid by insurance companies to think all that much. We are not paid much to diagnose complicated musculoskeletal problems. We are paid to fix things.

Physical therapy has now made their entry level PT degree a doctorate degree. For years they have been paid to fix things. They seek more. They too want to think (and be called doctor).

Massage Therapists have been paid to aid with massage (to do). They now seek more. Many already do manipulation even though I'm not sure they are really allowed to.

Chiropractic physicians are already trained as thinkers and doers. I guess we could spend a ton of money fighting against the broadening scope of allied health professionals such as physical or massage therapists who don't have anywhere close to our level of education. But I would suggest that a better idea, and possibly a battle we could truly win, entails us changing our image to be THINKERS AND DOERS -- experts in the conditions we

diagnose and treat (possibly with the option of using " drugs " if we so choose [please don't shoot me]).

V. Okay, I'll shut up (aka conclusion)

I apologize for the extremely long post, but I feel strongly about this very interesting point that Rick made, and the future of the profession. Chiropractic Medicine (title as justified above) has a lot to offer the general public and the health care (dare I use the word medical) field. Presently, the musculoskeletal specialists in the medical profession are orthopedic surgeons. Having studied medicine for the last 4 years I can assure you that they want to be SURGEONS, not physicians. They want to do surgery and not spend a lot of their clinic time providing non-surgical care of musculoskeletal problems. What that means is that the position of EXPERT of conservative (non-surgical) musculoskeletal diagnosis and management is VACANT. Do we want that job? We just need to say " yes " and adjust our image out of the alternative and into the medical field. It wouldn't really require change of what we do, but it might require a small change in how we think.

Even if DD Palmer wouldn't approve, its obviously not the rest of the medical community which is now keeping us from using the word medicine in our titles since they themselves (at Harvard of all places I think) are the ones who coined the term complementary MEDICINE.

The simple definition of the word medicine supports this addition to our titles. At one time it was necessary for us to step away from medicine or fear incarceration, but now may be the time for us to step back in. AT Still, the father of osteopathy, may be rolling over in his grave about the condition of the osteopathic medicine profession, but they have paved the way for us also to make this change. It is a matter of identity and name change or not it is about time we secure ours today and for the future (instead of allowing others to secure it).

These are just some things off the top of my head. Let's discuss this further. What do you guys all think? [i hope I'm not too new on this listserve to write something like this...ha ha]

Helllo Mike .. you are doing great. What i heard of hcg mail they are

very slow i am very leery of them . I and some others orfered from

drugdelivery i ordered mine yesterday. Where is Mass are you i have a

I ordered my hcg from hcgmail.com and I really wouldn't recommend

doing it. They are WAY more expensive then the other places I've now

found after the fact and they did take a while to get it here. It is

a legit company and I received Pregnyl from Organon through them, but

I would definitely advise finding somewhere else to save some money!!

I just ordered from drugdelivery.ca, but I haven't received the order

yet so I really can't say either way with them. They are A LOT less

expensive though. I received a phone call that they have billed me

and the order will be shipping shortly...I just ordered last

week...so far so good!!
