Yes, you're wrong. There are no guarantees in medicine, including fills. You were not paying for the fill, you were paying for the time, the staff, the office, the needles and such that were used up in the fill, and maybe even a little profit for the doc and salary for the staff. When you go to the doctor for an illness, he gives you an antibiotic, say, but that one doesn't work. So a week later you are still feeling crappy and he gives you a different prescription. Is the second office call free? No. Is the second prescription free, since the first one didn't work right? Do you expcet them to " eat " all these costs? Medicine is still a very inexact science, and there are no promises given. We all know fills can be very elusive, and can take several attempts to get " just right " for YOU. I would not expect to get a free fill or unfill. Certainly discuss this with the fill person, though. Many docs take too much out with an unfill. i've found that, if you get back very quickly for a small unfill, as little as 0.1-0.2 cc is often enough. If you wait any longer, much more is needed. But still, it is largely a guessing game - Also, another fill yet this week is, IMO, way too much trauma for your stoma in a short time. every time a fill or unfill is given, the stoma is irritated and traumatized for a day or more. But most people are worried about the first one. So i recommend asking for some numbing medication the first time, at least. that will sting for a second, but then you're numb. Then you can see what a fill is like, and see what s abig deal is is NOT. :-) You're a bit unusual, in that you have both a band and an RNY, so we're in kinda new water here! :-) You'll helpm us lear a lot. It would be useful (for me, at least) if you'd ask your doc how big your pouch is. A typical band pouch is about 3 oz or so. A bypass pouch is only thumb-sized - much smaller. I'm hoping your new pouch is band-sized , so more of the band guidelines apply, and we can help you more. Most of us get the soft stop signs only after a decent fill, so I'll guess that your hiccups now have more to do with eating a bit too fast and simply swallowing some air. That protein drink should take about 2-3 minutes minimum to drink slowly enough. And be sure not to shake it up wildly - that adds a lot of air that will come back up as a burp, and they can be painful. If you get a single hiccup after only a few bites of food, that should not be the " soft stop " sign - if you stop then, you'll starve! :-) How is your weight loss? Are you comforable after maybe 3/4-1 cup of food? If so, you may not even need a fill et. they are not given on any schedule - some need a first fill at about 6 weeks out, others do not. Don't want to be a pin cushion..lol I will definately as the Dr. what my pouch size is.. I thought he said it was about 30cc. I feel I can eat a fair amount of food especially if I don't pay attention to my body. I will post my menu for today and you can see my intake. I take my time when I drink my protein.. I make it warm like hot cocoa.. Drink it in about 10 minutes.. I could very well be eating too fast.. That has been hard to change but I'm working on it...I usually only get 1 hiccup... It's very weird.. I just laugh.. If I just started eating I try and slow down.. If it seems like I've had enough food I just stop eating...I will ask the Dr. about that as well.. Menu 1 35 gram protein Shake(prepared with 7 oz water) Breakfast 1/4 c. potatoes with bellpepper and onion 1 scrambled egg sprinkled with cheese Lunch 1/2 c. 99% fat free chili 1/2 oz cheese 10 mini triscuits 1 35 gram protein shake same as above Dinner 2oz. lowfat turkey keibalsa 1/4 c. scallop potatoes 1/2 c. Broccoli with carb options cheese sauce 1/2 dill pickle spear 1 hour at gym doing water aerobics 1 small apple with 1tbsp carb options peanut butter also had 10 fritos while helping son with homework and 4 bite size prunes to help with the constipation I get from all the darn Iron I have to take... I usually try to limit my carbs.. I try for 1 day unless it's a veg. so I went over a bit today...How does my menu look? what changes would /should be made.? There is a lot to learn that cannot be possibly learned in only the first few weeks or months. YOUR part of the " success formula " needs to be in place for a good loss, so continue to impreove your parts, so that when you get to a good fill, the serious weight loss will atart. Try to eat good bandster foods and use fitday to track your calories...it does help stay on track. I've gone through a fill or two that didn't seem to work at first, only to have them work REAL WELL after a week or two. Take your time and have a little patience with the fills, sometimes it does take several to hit that sweet spot. Right now I'm about 2.0cc's with a 10cc band and I can eat 1/2 of a sandwich in the later part of the day when I get looser but I stay full for 3 or 4 hours after I eat. We made an appt. for 3 weeks out from my last fill. So I am a week short of what you say is so important. (Oops!) Will you please explain to me how a fill can 'change' over a period of several weeks? I want to understand the concept of what the fill does (or doesn't) and in what way it could change so long after getting a fill. I do think I would wait the 4 weeks just to make sure. Sandy is very knowledgeable and has had lots of experience, along with medical training. With that said, I'm not sure who you talked to but if Dr. Kuri or Espinosa said it is ok, I would be comfortable with that also. This is not an overnight thing. Some people do not feel restriction until their 3rd or 4th fill. Please be patient and give this a chance to work with you. I feel sad that you are feeling this way. Go and get your next fill and give it a while and get yet another one if you feel you need it. When you get restriction, you will definately know it. I went to dinner last night and I was only able to eat about 3 oz of steak and a couple of bites of veggies and two bites of potatoes. This is about 1/8th of what I used to eat. This does work but you need to give it a chance. Heck you are hardly out of the healing stages. Even though some docs are incredibly rigid and anal on timing, many aren't. Your fill changes from day to day. Many of us are tighter at different times of day, often in morning. Women are often tighter or looser at certain times of the month. I can vary tightness by my consumption of salt....if I have too much salt I swell up and get tight. Also, the stomach inside the band is moving around in there, and like a pants leg with your hand around it, even at a given tightness the folds of cloth can leave different gaps or space. Guest guest Posted March 5, 2007 Report Share Posted March 5, 2007 It's not that the fill changes, but it's well-known that healing takes about 6 weeks, and stressing the healing and band with fills before it's fully healed is not a great idea. There are also several studies showing that fills before the 6-week period, which is what Inamed wants, increases slip rate later. You can try to test this if you want, but I would not for myself. I wouldn't want to be worrying lalter, with more and mroe fill, that the possibility of a slip was increased from starting early with fills. Is a week early for the fill worth concern for years? For ME, it would not be worth it. One of the MOST impt concepts with the band is that this is NOT a race to lose as much and as fast as we possibly can! the people who try to do this invariably get in trouble, and most end up much furhter bhind than if they had gone alsong slowly and steadily Some even lose their bands. We have all lost the FAST way before, and seen the consequences - wt regain. This time, let's do it right and avoid problems! I was apparently in a stressed-out and negative place when I wrote the question, and I am feeling much better now. I am making every effort to follow the band rules even though I know it's going through the stoma too easily. I'm putting less on my plate and concentrating on the small bites and chewing, etc. so that during this strange time I can at least practice my proper eating habits. I am scheduled for a fill on the 15th, which is exactly 3 weeks after my 1st one. Try to eat good bandster foods and use fitday to track your calories...it does help stay on track. Even though some docs are incredibly rigid and anal on timing, many aren't. Your fill changes from day to day. Many of us are tighter at different times of day, often in morning. Women are often tighter or looser at certain times of the month. I can vary tightness by my consumption of salt....if I have too much salt I swell up and get tight. Also, the stomach inside the band is moving around in there, and like a pants leg with your hand around it, even at a given tightness the folds of cloth can leave different gaps or space. Guest guest Posted March 9, 2007 Report Share Posted March 9, 2007 You're basically both a bandster and a bypass client, so pretty unique. i'm afraid pure band rules, or pure bypass rules will neither be just what you need. We can make suggestions, but you'll need to be sure your doc approves. I just don't know bypass needs at all. First of all, you're eating far too little for a Bandster to lose well or safely. We need about 1200-1500 cal a day, including good protein, fiber, and calcium foods. But YOU may need much more since your intestines are not absorbing well (how long out from your oritigal bypass are you? Were you gaining a lot(meaning your intestines had grown back)? There should never be pain with eating. What you describe is a classic band problem - " first bite syndrome " (esophageal spasms) that occurs only with thefirst few bites. This is painful and unnecessary. We never want more fill until this is solved, becuase we just cannot get enough nutrition in. We are never trying to eat as little as possible. This is the fix for most people: First, be certain that you are getting the minimum 64-100 oz of no-calorie liquids a day. Then, right before a meal, drink 1-2 cups of hot tea or another hot liquid. I'd avoid coffee here, as it is a gastric irritant. The hot liquid will relax and open the stoma. For the first week, you might want to also place a warm heating pad on your chest for 20 min before a meal. Then, have something " easy " like 1-2 tsp of yogurt or applesauce, to kind of " prime " the esophagus. Thrn, be sure you're carefully following the Band rules for eating. These will be essential so you don't have eating troubloe of barfing, which can cause band slips. If you're still having trouble, you'll need a small unfill to allow the esophagus to reast and get used to the fill. As little as 0.1 - 0.2 cc can make all the difference. Bandsters need to avoid all liquid calorues, uncluding juices and protein drinks. One of our basic rules is " avoid liquid calories " asince they don't make the band work. But YOY may need protein supplements if you're still malabsorbing. You'll need to be guided by your doc on this particular point. You never want to try to keep a too-tight fill. After a fill and a day of liquids, then a day of soft foods, we sshould be able to eat a decent small meal of solid meat and veggies within 5 days max. If we cannot, the fill is too tight. It's easy to stretch the band pouch pouch with a too-tight fill - just as the bypass pouch is easily stretched - so it's impt to never keep a too-tight fill. Some of your food choices are not the best. If we want to lose and do wewll, we need to greatly improve eating. Cheeseburgers are not good protein, usually gristl-y and hard to chew, and poor quality meat. The bun will never work with a good fill, and bread is useles nutrtion. further, it swells in teh pouch and can cause trouble and dilations. Coleslaw is hard to chew, and usually high-cal. Later, with eating experience, it might be ok sometimes. You might look in the files or database here for the document called " Band eating rules... " . It covers all the basic band needs re nutriiotn,, etc - but, again, you may need to gete clearer info from your doc. We;ll try all we can to help you, but some of the band rules cannot apply to you. It just seems that every day I plan on going to Curves something happens to take my time away from me. I take care of my 95 yr. old mother, and it's a lot of work and Dr. appointments, etc. I know that's an excuse, but I just haven't been able to balance my time very well. I need to work on that! I decided to go to TJ for the fill...even if it turns out that I don't need one, I will feel better having seen the fluoro so that I can psyche myself up to do whatever I need to do. I really need to monitor my food intake for a few days at Fitday. I go up and down with the same 2 pounds every other few days, so I know it has to do with the calories. I guess I just need to kick myself in the butt after tomorrow (Tijuana) and get serious with the program. Thanks for all the good advice you always give to folks. Guest guest Posted July 19, 2007 Report Share Posted July 19, 2007 Reporting on the fill: I saw Dr. Morales yesterday in TJ. The fluoro showed that my band is fine, and in the right place. That is comforting to know, and I am happy that I am able to get to TJ and see the ex-rays, as opposed to those people in other states who see fill Dr.'s that do not do the ex-ray. Although the restriction was 'pretty good', Dr. M decided I could have a little more, so she put in just .4cc...........I am hoping that will be just the amount I needed to get to my sweet spot. I know others, such as Elaine, have had very small fills that made a HUGE difference, so I'm hoping that will be true for me as well. One thing that was GREAT about the trip: Just being on water all day instead of eating, helped nudge my weight loss into a 'go' position again...I dropped 3 pounds, down to 209. I know that it probably will go up again slightly when I start eating again (soft food this afternoon) but from a psychological viewpoint, it encouraged me to see that this WILL work for me when I follow the rules to the letter! I haven't been down to 209 in 18 years! My doc does his first fill at 6 weeks, but there was a conflict with my schedule and they did mine at 5 weeks.
