Wisconsin cochlear implant mandate

I am confused by all these statements about Wisconsin being the first state to

require health plans cover cochlear implants.

 

Kentucky did this back in 1998, correct?  Has something changed?

 

FYI, Utah and Idaho insurance departments both issued regs excluding cochlear

implants from coverage under individual and small business plans.
