Note about Hearing Aid Survey

Note: If your hearing loss occurred during your childhood or teens -- or if

you haven't purchased a new set of hearing aids within the past 5 years, you

will be disqualified from the survey. I was disqualified given the fact that

my last set of hearing aids was purchased in 1996 and the onset of my

hearing loss was age 3.

Right ear - Nucleus Freedom

Implanted: 2/1/06 Activated: 3/1/06
