What I have found (and you'll see in the files section - numerous

samples of diet progressions, protein recommendations) that most of us

recommend 60 gms a day for women and 75 gms for men. Fact of the

matter is you have to realize that most pts have to work real hard to

eveven get to these levels. So I just focus on nutrient density,

adding protein powder to small serving sizes (beneprotein is great and

the rep always sends me samples). It is like baby steps to get to

their goals. If you use a " formula " you probably won't get a realistic

goal for this popn.

As far as extending pureed stage - go for it if pts will comply! It is

very hard to get them to do this for a month. I think one month seems

sensible and then really emphasize the soft, moist, chewing throughly

after that, etc...

Thanks Theresa. The recommendation here has been 70 -90 grams /day

and they only count " high biological protein " . I haven't found

patients who can eat a balance and get this much protein in. I also

felt that this was high for some of our patients.

Our protocol is now 4 weeks of pureed and I truly question

compliance. I feel that compliance would be best if the pureed stage

was shortened- thereby increasing nutrient intake/protein.

I have noticed that there is a lot of questions about how much protein we need so here is what I came up with... Active Adults 18-55 Should consume 0.4 to 0.8 grams of protein per pound of body weight spread over 5-6 smaller meals a day. Example 200lb x 0.5 to 0.8g protein / lb body weight = 100 to 160 of protein a day. depends on how active you are. Adults 55 and over should consume 0.4 to 0.6 grams of protein per pound of body weight depending on how active they are.. This should give a little better guide line to what your body needs in the way of protein...

We use 1.0 g/kg IBW for bands and 1.2 g/kg IBW for bypass.

I use 0.8-1 gram/kg adjusted ideal body weight

Not sure where those numbers came from-they were already established

when I got here. I'm assuming they just increased it from the

typically recommendation of 0.8 g/kg and decided that the bypass

patients would need more than the bands b/c of the more invasive

surgery.

I was at that conference last Sept and

that is exactly what they said… 70g/day to preserve lean body mass.

We use 70-80 grams on average. I do believe one of the main

reasons is related to most pts typically eat 1000 or less calories post

operatively. I BELIEVE I remember at the weight management conference

learning that when one consumes less than 1000 kcals daily they need 70 grams

typically as a minimum to sustain adequate visceral stores. (I would have

to pull my articles for confirmation) Again, this has been a few years

back, but is the premise for our program recommending 70 grms as a

minimum. If you use 1 gm per kg of IBW for women who are shorter that

would be 45-50 grms or less, and I don't think that would cover their needs

when they are s/p surgery and consuming about 800 kcal daily. I may be

wrong though...........any one else have thoughts?
