It's a term from when folks used the TDD or TTY devices to " hear " but could speak without problems. So a deaf person who is oral can speak into the phone, but the relay operator would have to type the reply for the Deaf person to understand what the reply was. When you call using the relay, you'd tell the operator that you use VCO and they just type the replies. The Radio Shack device allows you to read what is being typed by the relay operator, while you speak, or do VCO. Hope that explains it.
