My Fellow MGBers,

I am gathering stories from Dr. Rutledge's MGB patients for a

possible

book. I am attempting to compile a group of stories from people of

varying sizes, ages, genders, and occupations. It is my hope that

this

collection will be used to help others who are searching for the

right

solution to their struggle with obesity. Not every story that is

gathered will be used in the final collection. If you know someone

whose story is particularly remarkable, please ask them to share it

with us so that we can consider it for publication.

Following are a list of questions that I would like to have answered

by those who are willing to share. Not every answer will be used in

the final draft. I am simply attempting to gather the data and sort

it

all out. Please be patient with me on my journey of discovery.

Name? Address? Email address? Phone numbers? Age?

Occupation at time of surgery? Current occupation?

Date of MGB?

Weight at highest? Weight on date of MGB? Lowest weight post-op?

Current weight?

What health concerns or comorbidities, if any, did you have pre-op?

How has your health changed? What medications are you still taking?

Have you had any complications?

Have you had or do you plan to have plastic surgery?

If your insurance covered any part of your plastic surgery, what were

the circumstances?

Insurance or self-pay? Insurance company? Amount of coverage?

Reason for denial? Did you appeal? What was your insurance process

like?

How did you cope with the food prep post-op? How did the MGB affect

your work?

What has the impact of your lower weight been on: Your family? Your

friends? Your coworkers?

What has your favorite post-op moment been?

What was your least favorite moment post-op?

Would you do it again?

Where did you have the surgery done? What do you think of Dr.

Rutledge

and his staff?

What did you think of the hospital?

Is there anything you can think of that could be done to make the

process easier?

Would you be willing for this information, or your story, to be

released in book form as a part of a collection of MGB post-op

stories?
