or even you, to phone me. i beleive you have my home phone number.

Hi Dr. R

I just filled out that form, don't have time to do it again, but my

doctor's info is current and Yes you have my permission to contact her and

utilize

the info in any way that would be helpful. I'll let her office know. Jil A.
