Dogs are visual (i.e. they look at your face before responding to what they hear), so using sign or other hand commands helps reinforces this and allows the dog to focus more on you instead of auditory stimuli that can distract them. They say guide dogs (not sure if this is also true of hearing and other service dogs) have a vocabulary equal to that of a 7 year old child. When Tigger needs to go outside, she gives me her paw. The bathroom sign is something I'm going to try with my next guide dog. This hasn't happened often though and i've discovered that if he gets to walk enough during the day to get things going, thayt we don't have problems and i also discovered they were on my days off when i wasn't as likely always to go somewhere/.
