I know that we fill out the follow up form at 1 year but do we come into CLOS

for an exam/check up or anything? I know that we see our GP at 1 year for a

physical and blood work...I had it done at 4 months and had my results faxed to

CLOS and I will do the same in January at my 1 year mark. Thanks!

If this was addressed specifically to me, I did my follow-up form. In fact, I

called your offices and Penny said she was looking at my form at the time. I

didn't think it had processed correctly online, but she said she had it.

I'll be glad to send again if you do not have it.
