you have to call CLOS directly i believe and ask them for the appeals

letter.. also, if your interested... i had to fight UNITED HEALTHCARE for

reimbursement.. i could forward some letters that i sent them if your

interested!!

You need to request an appeals letter, send your request to CELOS, 925
