Any Recommended doctors List for ctoma surgery?

Did you all use the best local surgeon you could find for your removal surgeries, or are there some 'best' doctors that only specialize in ctoma removal that are worth the extra trips and money to go for the surgery? Any universally recommended names within this board?

It seems there is a big difference in terms of total removal as well as preserving hearing in the ear.Also after the surgery, any 'no flying' restrictions? For how long? How long after the surgery can you ride long distance in a car then?
