PT therapy for THR

I am curios what everyone does for therapy once they get home after

having their hip(s) replaced. I was given a script for 4 weeks 3

days per week. The PT is recommending that I get more once it runs

out next week. The outpatient therapy is less demanding than what I

had in the hospital. I asked the PT when it would advance and she

said it would stay the same.

She has me doing mat exercises to slide my knees up/down 30 times

and spread my legs out on the mat 30 times. Then a knee triangle is

placed under my knees and 3lbs of weight are strapped to my ankles

and I raise my legs 30 times with a 6 second hold. Butt squeezes

are also done and sometimes they do ankle exercises with a rubber

band. Finally they put me on a treadmill for about 10-12 minutes.

I do additional exercises at home that were recommended from the

hospital rehab. Using a my walker as an aid I raise my knee/forward

30 times, swing my legs left/right and raise my legs backward

without bending the knees.

I am wondering if anyone else has received different/more advanced

therapy as time progressed?

I guess it depends on your PT. I have a great place we have been working for 3 months now , 2 or 3 times a week. They have progresively made things more difficult and changed the exercises. Your therapist should listen to you if you think things are to easy. I have complained no end about my therapy but the bottom line is it is working. If they hadn't pushed me( and still are) I would never be as far as I am. I was shocked last week. I took my kids to a zoolights event. We walked for an hour and a half plus had a long walk to and from the car. I was tired but I didn't hurt. Before my hip I had half hour tops of standing before I was done in. Make sure that your therapist is working for you. It's your life and your recovery in the balance. Except during the hospital stay I feel that PT should be individualized depending on what each of us considers "fully recovered", where you were prior to surgery and how fast your body adjusts to the new higher stress level. You didnt mention how many weeks post op you are, but since you're on a treadmill I'm guessing you no longer have any weight bearing restrictions? I would not increase from what your doing until most restrictions are removed and your doc agrees with the progression.

I have no doubt that what you are doing is fine if all you want to do is walk normally. In my case since I want to bike, swim and cross ski I do more. After six weeks when my weight restrictions were removed I started working with surgical tubing (I bought a kit from Sportmart for less than $20, it has an assortment of bands and straps) to strenghten my hamstrings, quads, abd and adductors. I personally like biking better than the treadmill since the muscles are worked through a greater range of motion, its also involves less weight bearing. I also do quite a bit of stretching which you didnt mention. I focus on my hip flexors (upper quad area), quads, hamstrings I also do my calfs justs because it feels good. The one my doc wanted me to do was to; while seated place the affected leg so the knee would be pointed out with the foot in (sort of a figure 4) and push gently down on the knee. This drives the ball into the socket. You must at all time be cautious of your restrictions and the possiblity of dislocation when stretching.

My OWN guideline is that if you find you're completing your sets easily AND there is no residual soreness the next day you can increase, first I would suggest increasing the number of reps, but since you're already doing 30 I would start incorporating weight AND reducing the number of reps. Can you stand and lift your leg to the side (abductors) or have you only do this while lying down? If so I would progress to standing with fewer reps: say two sets of ten and see how that feels. The goal should be to do all with good FORM; that is a slow controled leg lift throughout its full range BOTH up and down.

I checked with my insurance and was able to get another month of therapy. I

didn't even use all the visits I could have gotten. The leg raises to the

side and back I did like you do until I got to ouside therapy and then I

used a machine. I also strengthing my calfs on a machine where I sat and

used weight in two diffreent excercises to lift and push, from the knee

down. These were added after I did the ones you discribe on the mat for a

few visits. You may be in better condition then I was. I had a very weak leg

from years of limited mobility and not being able to excercise or walk very

much.
