Zantac/miochol C. Inderal/hyoscamine D. glycopyrrolate/ranitidine 2. A. Antihistamine/antibiotic B. H2 Antagonist/Cholinergic C. beta blocker/belladonna derrivative D. antimuscarinic/H2 Antagonist What do you think the correct/best answer is? If we see H-Pylori bacteria involvement which is commonplace, then an antibiotic such as Clarithromycin is used along with medication to inhibit the secretion of stomach acid from the parietal cells such as an Histamine 2 or H2 antagonist or blocker. Older medications such as your anticholinergics work as well. If we see no involvement of H-Pylori, then an H2 Blocker and anticholinergic or antispasmodic would work fine. So without being specific enough I would use 1. D or 2. D as the majority of antimuscarinic's are anticholinergics. Great Question! (1.D and 2.D) Allow me to say however, without the diagnosis or type of GI ulcer written into the question the pharmacy technician should still be able to pick out the BEST possible answer for a GI ulcer. Since the technician should also know causes, factors or types of GI ulcers, he or she should be able to pick out the combination listed that works, as only one combination does. Process of elimination can also be used on a test, but on the job the tech should know which drugs are used for which disease states. I liken this to my using the word 'antimuscarinic' in my question,as I teach my students that either antimuscarinic or anticholinergic can be used, yet I usually use the term anticholinergic. They still have to know it! To include a H,Pylori in the question, which is the MOST common cause of GI Ulcers, the possible answer set would have to include a PPI and a 'Macrolide' Antibiotic that is used in Dual or Triple Therapy for GI ulcers caused by H. Pylori. Some GI ulcers are caused by stress, use of medications such as NSAIDS when COX I is inhibited and other reasons which do not include H. Pylori. In this case the original questions and answers of 1D and 2D would prevail. Here is an other version of the question with the specific type of GI Ulcer mentioned: 1. Which would be used to treat a GI Ulcer caused by H.Pylori: A. Zyrtec/penicillin B. Zantac/miochol C. Inderal/hyoscamine D. omeprazole/clarithromycin 2. A. Antihistamine/penicillin antibiotic B. H2 Antagonist/Cholinergic C. beta blocker/belladonna derrivative D. proton pump inhibitor/macrolide antibiotic In this case with THESE questions and THESE sets of answers the answer is D (1. D. and 2. D) Since H. Pylori is an infection an antibiotic needs to be used to eradicate the bacteria. However it must be a tried and true Antibiotic. PCN does not work in this case. YES, newbies, the tech should know this. Thus A is not the correct answer. Newbies or those studying for the PTCB exam please feel free to post your comments or questions.
