If you believe your pharmacy or pharmacist has has problems with the 'endings' of drugs causing medication errors please pass this information on: Post it or give it to your director/pharmacist. " Introduction The National Coordinating Council for Medication Error Reporting and Prevention (the " Council " ) is asking for your input regarding the use of drug name suffixes. This survey has been disseminated by multiple organizations so if you have recently completed an on-line survey sponsored by the Council about the use of suffixes or from another organization, message board, or list serve, you do not need to complete this survey again. The intent of this survey is to assess practitioners' experience with the use of drug name suffixes. The results of this survey will be used to assess the current use of suffixes, problems that may arise from the use of suffixes and how those situations are handled. The current practice of naming drug products within the same product line (e.g., different strengths, formulations) is to add a suffix to the existing name rather than propose an entirely different name (i.e., " Drug name SR " ). Suffixes have also been used to describe many different and unique characteristics of drug products such as distinguishing a new modified dosage formulation from a currently marketed immediate release or modified release dosage formulation. A suffix may be a letter, number, a word, or combination of letters and/or numbers attached to the end of a proprietary drug name. They can range from one to four character strings of numbers or letter than don't spell anything, or a suffix can be a word or a word part that describes the package, a package characteristic, or a product characteristic.
