Dear Certified Pharmacy Technician: This e-mail is being sent as a reminder that your CPhT status expires on July 31, 2005. To ensure that you receive your new certificate by your expiration date, it is advised that you submit your recertification application and fees by June 13, 2005. Individuals who opt to recertify after this date will still receive a new certificate; however, it may not received before the July 31st expiration date. The fee for technicians who recertify online at www.ptcb.org is $35, and $50 for those technicians who apply for recertification using the paper application. The online recertification process has been available for PTCB technicians since 2001 and has proven to be an effective tool for technicians to reduce the time it takes to complete the recertification process and receive their new certificates. By accessing your MyPTCB account at www.ptcb.org you may obtain your recertification status, update your address and contact information, and complete the online recertification application. Free online CE and the ability to print a paper recertification application is available on the PTCB website as well. Technicians who utilize the online recertification process benefit by: • Immediate e-mail confirmation of submission of your recertification application; • E-mail notification when your recertification application has been approved and processed; • A $15 discount on the recertification fee • Ability to pay the recertification fee with a credit card or check; and • Receipt of your certificate up to three (3) weeks sooner than the paper recertification process. A postcard with your MyPTCB password was sent in December and a paper recertification application has been mailed to the address we have on file earlier this month. It is important that you keep your street and e-mail address information current with the PTCB to ensure delivery of your recertification materials. If you did not receive your MyPTCB password or have misplaced it, you may obtain it by calling 800-363-8012 or by clicking on " Password Help " in the MyPTCB section of the PTCB website. Requests for name changes must be faxed to PTCB at 202-429-7596 and must be accompanied by appropriate documentation (copy of marriage certificate, divorce decree, etc.). Recertification applications or corrections are not accepted via fax. Please share this information with your fellow CPhTs and post this message at your pharmacy.
