This was question 123 on the PTCB practice test. I posted, How much 1:1000

stock solution is required to prepare 500 ml of a > 1:400 solution? Answer

is 125 ml.

I did make the mistake of omitting a zero. It should have been 1:4000

solution.

So by doing it this way 500/4000 = 0.125

0.125/x =1/1000 =125ml

I see now how to work out this type of problem.
