So the order is 1000 ml NS (1L)w/ 20 mEq KCl infused at a rate of 125ml/hr. To start off, we need to see how long 1L of the NS Vessel would last. So it's 1000ml/125ml and we would get 8 hours. One bag would last 8 hours and a day is 24 hours, so we would need three bags of 1000ml NS. Answer a. 3 bags of 1000ml NS Next we are told that we have in stock 2mEq/1ml of KCl in a 15 ml vial (Note that these vials are single dose vial[sDV] meaning that they can only be punctured once)and the order was for 20 mEq per bag. Since we now know the number of bag we need, the amount of KCl we would need can be solved by 20 mEq/bag X 3 bags = 60 mEq KCl. Next we need to determine how many vials we would need, so since we know we need 60 mEq KCl for the three bags, we can used ratio proportion to solve the amount that we need. So we use what we know, 2mEq/1ml (from the availability) and set it equal to 60 mEq/x, it would look like 2mEq/1ml = 60mEq/x, and when we solve for x (cross multiply), we would get the result of 30ml. So each vial is 15 ml each and we need 30ml, 30ml/15ml would yield us 2 bottles. Answer b. 2 vials of 15ml KCl Lastly comes the syringe, syringes come in 1ml, 3ml, 5ml, 10ml, 20ml, 30ml, and 60ml from what I remember (There may be more). Since we need 30 ml syringe to withdraw from the two vials of 15 ml. We have solved Question #2 for you and we will go in detail on how to obtain the answer. First, the R/x is for 12 fl. oz. We will first change this into milliliters using the conversion factor, 1 fl. oz/ 30 mL. You should get 360 mL. Next, use ratio-proportion math to convert the quantities for the 240 mL ingredients into the 360 mL ingredients. Tetracycline: (16 caps/240 mL) = (x caps/360 mL) x=24 caps TCN Hydrocortisone: (15 mL HC/240 mL) = (x mL HC/360 mL) x=22.5 mL Hydrocortisone Lidocaine: (30 mL Lidocaine/240 mL) = (x mL Lidocaine/360 mL) x=45 mL Lidocaine Lastly, you would QSAD with Mylanta up to 360 mL. I do not know if it will be on the exam, but apparently who ever asked it found it in a study guide book. These are questions that I teach my students to answer because you have to be able to do this to make IV's. So I HOPE it is on the exam. If it is not I would say the exam is lacking basic IV mfg competency. However I am not an item writer.
