Start out with the Institute for Safe Medication Practices (ISMP)

at http://ismp.org

Most of the pharmacy journals have an area dedicated for medication

error discussion, and will have a CE on some aspect of errors at

least once or twice a year

There are also many CEs available on the topic---some free, some for

a fee

RxSchool has several free CEs. You'll need to register (free)
