In my neighbor's case, it wasn't caught because everyone was looking into her speech, not her hearing..when she was very young, she would pass all the regular hearing tests that one could do on a young child. She was very good at reading lips too, therefore she fell through the cracks. Auditory difficulties and deafness was covered medically. I have another friend whose child had anger problems as a very young child. When he was 3 they found out he was deaf. Deafness can show itself in many ways.
