My rheumy allows me to taper and increase if necessary with flares - but I think the goal is to reduce so that you do not flare big time- and I think most here - can tell the difference between a real flare and return of discomfort. If you aren't sure what he means - I think it is okay to call and ask - I really don't think he is saying he wants you to reduce until you flare and then up four ( if that is what he means - you may want to ask what that achieves???). I would be especially careful because of the pericarditis. I am sure the pred is meant to reduce the heart inflammationa as well - and joint inflammation is horrible enough without risking the heart inflammation too. Better not to guess or to let us guess with you - no matter how well intentioned. Good luck in the reduction- sounds like you have toughed out of a lot of the rough stuff already.
