Recombinant Human Insulin wasn't always available. Pork insulin is only one

chemical flip from human insulin, hence it had the least chance of causing

an immune response for humans.

Since the creation of the recombinant human insulins (Humulin, Novolin, etc)

then the majority of the Pork products have been either discontinued or are

harder to get a hold of.

In addition the reason why a person may need pork is if they are

allergic to recombinant or beef insulin. While this is rare, it does

happen. My neighbor is one of them! Go figure! :)

However it is getting difficult for her to find it. Because of this

I would not doubt that it becomes an orphan drug soon.

Jeanetta,

It can still be compounded. :) Check for veterinary compounding specialty

shops, especially, as some cats don't respond as well to recombinant human

insulin as they do the pork insulin. So there are compounding pharmacies

that do make the PZI insulin for humans AND pets.

Thanks Della,

I never knew where she was getting her insulin from.... But I will

make it known to her just in case she needs to have an alternative

supplier. I know she would be so happy. She has

Charcot's Disease with a double thigh high amputee on top of

Diabetes! Weighs over 200 lb! Amazing to see her get around in and

out of bed, tub etc!

I was listening to the show when a man called in about being allergic to his insulin preservative last week and did not know what to do. Later that week I went to a CE program where it was announced by the proud drug rep. you could leave the insulin out 42 days because of the preservatives which he "had no idea what they were". Of course it is written on the side of the box--It is creasol (A VARNISH STRIPPER ) and phenol!!! One of the pharmacists told the speaker (who knew about the preservative) a lot of people are allergic to creasol and the speaker said it is a big problem! I later told this to a friend who is an MD and sits on a diabetes committee at a hospital about this, and he found out that a patient who had dropped their insulin and it broke--well the insulin ate a hole in their linoleum !!! People can't live without insulin, I guess diabetes is getting too expensive?As I have spoken about this to other doctors, they are finding that TPN and other IV's are causing allergies they have never seen before and they have been in practice for 30 years

This is really really crazy.

I was listening to the show when a man called in about being allergic to his insulin preservative last week and did not know what to do. I was listening to the show when a man called in about being allergic to his insulin preservative last week and did not know what to do. Later that week I went to a CE program where it was announced by the proud drug rep. you could leave the insulin out 42 days because of the preservatives which he "had no idea what they were". Of course it is written on the side of the box--It is creasol (A VARNISH STRIPPER ) and phenol!!! One of the pharmacists told the speaker (who knew about the preservative) a lot of people are allergic to creasol and the speaker said it is a big problem! I later told this to a friend who is an MD and sits on a diabetes committee at a hospital about this, and he found out that a patient who had dropped their insulin and it broke--well the insulin ate a hole in their linoleum !!! People can't live without insulin, I guess diabetes is getting too expensive?As I have spoken about this to other doctors, they are finding that TPN and other IV's are causing allergies they have never seen before and they have been in practice for 30 years

This is really really crazy.

Dear Crstal,

I think you are refering to the 1500 or the 1700 rule that helps one predict how

much insulin to ADMINISTER to the patient:

1500 Rule

A formula used to predict how much one unit of

Regular insulin will lower blood glucose. 1500

is divided by the total daily dose.

Example: If total daily dose is 50 units:

1500/50 = 30 mg/dL

1.0 unit of insulin drops blood glucose by

30 mg/dL

1700 Rule

A formula used to predict how much one unit of

rapid acting insulin will lower blood glucose.

1700 is divided by the total daily dose

Example: If total daily dose is 50 units:

1700/50 = 34 mg/dl

1.0 unit of insulin drops blood glucose by

34 mg/dL

The first thing I thought of is the cartridges - they come in a box of 5 x 3ml

cartridges which is a total of 15ml of insulin which is 1500 units in total. We

seem to dispense a greater number of cartridges now and only a few of the

vials... Most people have insulin pens for easy administration. Pumps will use

vials but they are still relatively uncommon but I'm sure we'll see more of them

in the years to come.

lasts it is 300 units. But if they are asking how long a BOX of 5 pens lasts

then it would be 1500 units. So what is the most common purchase:  A

pen/cartridge at a time or a box of them at a time. 

So as a Pharmacist, can you explain to this forum how you explain to your techs

how to calculate days supply on this item?  The site says one pen  can last 14

days once open and one vial 28 days. Dose any of this figure into the days

supply?

We only sell the cartridges in the full box of 5. It depends on the insulin in

the cartridge - some only have a 10 day expiry at room temp (once they put the

cartridge in the pen it is stored at room temp while the rest of the cartridges

in the box would be kept in the fridge.) They very rarely need a new pen -

maybe once per year but the hospital here gives them the pen on discharge in

most cases, and since we are a hospital outpatient pharmacy, we very rarely

dispense pens. Someone from a community pharmacy may see more pens than we do.

If the cartridge would last longer than 10 days (if using less than 30U per day)

than you would calculate each cartridge at 10 days and the box would last 50

days. Because the different insulins have differing cartridge expiries (most in

the 10-15 day window) we use 10 days for all of them as it easier to remember.

If they would use less than 5 cartridges in a month but their plan only pays a

30 day supply, we would round down to 1 box = 30 days. If the plan will pay

more than 1 month or the box would last less than 30 days (depending on the

dose) then we would calculate the actual days.

Eg. Lantus patient using 55 units SC QHS = 1500 Units (in box) divided by 55

units per day = 27 day supply.

It is much harder if the patient is using the insulin for a sliding scale as you

really don't know how often they will need to use it or how much at each time

they will need so we often give 1 box = 30 day supply unless they start going

through it faster than 30 days and then we'll adjust to match their usage.

in my setting we will separate the cartridges from the boxes if the usage shows

that a month supply will be more or less than a box.
