Patty,

I have heard that methadone is great at relieving pain

that other pain meds don't. However, it is a very

stigmatized drug, and usually only used for heroin

withdrawal. I know in Denver, there were only 2 places

you could get methadone, and they were heroin withdrawal

clinics, and had to have special licenses to administer

methadone. No scripts were given to pts--all pts had to

come to the clinic to get the med. No docs, and I mean

no, dispensed methadone.

Then I came to Seattle, where the drug of choice here is

heroin. The hospital I work at is the county hospital

and the regional trauma center for 4 states. We get

LOTS of heroin users, and it seems like every other pt

is on methadone. It really blew me away. Then, I was

taking care of a 28 yo woman who had chronic,

intractable pain. I had given her a total of 75mg

morphine IV, 4mg Dilaudid IV, 500mcg Fentanyl IV, and

75mg Demerol IV, all without much relief. So, I called

the pain service and had them come down to evaluate

her. The doctor looked at what I had given her and the

results, and he put her on a Dilaudid PCA, and oral

methadone. Methadone is a pain med, but I've never seen

it used for pain. So, I asked him about it. And he

explained that methadone is not a drug commonly abused

because most docs won't write for it, and because

several states require special licenses to dispense it.

Therefore, most people don't go requesting methadone,

and most don't know that it is a pain med. They haven't

developed tolerance to it like they have other pain

meds, and it works on some different areas in the brain

then other pain meds. 2 days later, I asked the pain

doc about this pt, and he said that the methadone had

adequately controlled her pain, and she was feeling much

better. Just this past wk, I have taken care of 2 pts

postop who were on methadone for pain, and who were not

heroin addicts.

I'm glad you found a doc who was willing to prescribe

it. Unfortunately, because of its association with

heroin, I think it will always have that stigma, but if

it works, and your doc is willing to rx it, then my hats

off to you and to him!

Let us know how it continues to work for you. I will be

curious to see. Maybe I should start a study here in

Seattle with methadone.....
