In Texas, you can either go directly to an health dept location and pick one up or request it through the mail. Then, once you get that form....that is when you decide how you want to fill it out and get it notarized. I found it was much more simple than I ever thought it would be to obtain the form itself. My son was vaccinated through 18 months as well, and my daughter through six months. In Virginia, vaccines are listed on the same form as the physical (vision, hearing, urine, etc.). What do you do in that case? Can you submit a physical form with a few vaxes listed, along with your exemption? BTW, the Virginia link on the National Vaccine Info Center web site is out of date.
