You should be able to contact your state health department and they

can direct you on how to get one. In Texas, you can either go directly

to an health dept location and pick one up or request it through the

mail. Then, once you get that form....that is when you decide how you

want to fill it out and get it notarized. I found it was much more

simple than I ever thought it would be to obtain the form itself.

However, explaining what the form is to people once you start trying

to use it is a completely different story. Good Luck!

I would never mention any vaccines at all.

They won't have her medical records.

every state is different

Most states it is NOT thru the health department

You have to know the state and what you do is not applicable to other states.

Yes, but what if the school requires a physical form? My son was

vaccinated through 18 months as well, and my daughter through six

months. In Virginia, vaccines are listed on the same form as the

physical (vision, hearing, urine, etc.). What do you do in that

case? Can you submit a physical form with a few vaxes listed, along

with your exemption?

BTW, the Virginia link on the National Vaccine Info Center web site

is out of date. Here is a link I found that works:

What wouldn't work is if you still give some and avoid otheres.

Can anyone help?

Florida requires a signed application form in order not to vaccinate. This form is only available for kids and one must make an appointment to see the nurse!

NEED: something like an Affidavit of Waiver against Vaccines.

I tried to contact the FL State Health Department with no response.

Any help is appreciated.
