What, does he think you should just stop after 5

months!! My doctor that I used to see, who was very

good, quit too. What I soon realized was that he was

the only doctor in that group who really knew anything

about HCV. He had, until then, handled all their HCV

cases. When I saw one of his ex-partners he told me to

come back in 5 years, maybe there would be something

new by then! Translated, that meant 'I really don't

know anything about HCV and really don't want to

learn! Go Away!!!' No, I say dump the doctor, not

the combo. I'd get on the phone first thing Monday and

start calling around looking for a new GI. Don't you

have a support group? Maybe they know someone. Call

your insurance co. To stop now is the same as them

throwing away all the money they have spent so far.

Good luck!!

Alley.

Look what happened to me when I was pulled at 5 1/2

months. My Viral load shot up to 29 million in 4

months from 350 thousand. Do you get mail from Hep C

info? there is a link on the messages to go to there

home page. It has a list of good Dr's maybe some in

your area. DO NOT STOP. The Dr who did my last viral

load was just about as competent as the one you are

talking about. He told me to just wait till something

new came along because I was a non responder after 5

1/2 months. I responded I went from 12.5 mill to 350

thous I am a 1a which needs to be treated longer than

6 months. I was mad as hell at him too....Take Care

I wrote to Sheltie House but have not received a reply

but left my address so maybe they will mail me an

application, Thanks Again, Connie

Thanks yall for being mad with me hahaha.

Man, crackers and 7 Up are the meal of the day today. After having that

sinus/migraine wed and getting those shots at ER, then dealing with this

backwoods gastro (I'm trying to be nice and not cuss haha), I ended up doing

the throne thing all nite and day. Ah well I needed a good cleaning out!

Yes, I will not take this lying down. I will have to do my gastro shopping

all over again. I've got a little over 2 months worth of combo left. My 6th

month PCR is in Nov.

Question - should I go ahead and do my PCR labwork now, instead of waiting a

month? If I'm PCR negative, as a genotype 1B I would continue treatment, if

I'm PCR positive, then it's a moot point to continue.

What yall think?

Obviously this physician is uninformed about the current research studies

that provide massive information regarding " liver function test and their

significance in treating HCV. " (Claudine is so very right when she says this

doctor is clueless!!)

Quoting just one recent study (although there are several dozen I could have

chosen from!) .....

Significance of baseline serum alanine aminotransferase (ALT)

levels. The demographics of patients with mild chronic hepatitis C

have not been well characterized. Stuart Gordon and colleagues

identified 105 individuals with baseline serum ALT levels £1.3 x

the upper limit of normal (ULN) from a pool of 1,744 patients with

chronic hepatitis C who had taken part in antiviral studies.

Although lower baseline histologic activity index scores were found

in these 105 patients, the baseline serum ALT level was not related

to either serum viral levels or HCV genotype. Also, no difference in

sustained response to antiviral therapy was found between patients

with baseline serum ALT levels £1.3 x ULN and those with baseline

serum ALT levels >1.3 x ULN. These results suggest that patients

with minimally elevated ALT levels have milder histologic disease but

a similar sustained response rate to patients with higher ALT levels.

(Gordon SC, et al. Hepatology 2000;32:400-404)

We who study patients who are in end stage liver disease see no correlation

between the standard acceptable ALT and enzyme levels and the patient's true

liver health. This is one of the reasons that WE SUGGEST EVERYONE GET A

LIVER BIOPSY! However evasive some believe it to be, unfortunately this is

the best diagnostic tool available. This will change in the future! There

are better MRI/ultra sound type machines being developed to aid in diagnostic

capabilities. The future is looking brighter.

We also are fighting to make sure that EVERY HMO or managed health care

system realizes the potential in delaying these important tests and

treatments.

Sure your viral count may not be undetectable right now, but the research

shows that at least your probably not progressing onto the next level or

stage of liver disease while on combo.

What I fear most is that your new 'DOC' may have a clue but is trying to save

money. Some insurance plans give cash or other incentives for Doctors to

delay treatment, even wait for a patients death (in some cases) hoping to

save on their quarterly expenditures. Then last month I saw an internal memo

from a very large health insurance company who said they are wanting patients

to wait for PEG/Ribavirin to be available. And I quote... " Since over 60% of

Hepatitis C patients identified by enrollment statistics have not benefited

from the current mode of therapy, REBETRON, it is advised that physicians

delay treatment until 2001 "

Now since this was given to me by a discruntled patient, I am uncertain of

it's authenticity, but I don't doubt this is the thought process of many

patients and physicians from talking to many of them about their frustrations

with Rebetron and anemia.

>>>>>>I'm in my 5th mo. So I should just dump the combo and go back to

feeling like I'm 100 years old again? <<<<<

What I suggest you do if you want to continue on Rebetron is go to the

Administrator if your health insurance company and respectfully, but sternly,

suggest that you need to finish your 24 weeks or 48 weeks (which ever you are

under) of combo that was prescribed by your former doctor! This is the

health path you want to continue with and by law should be able to do! Start

by calling them. Then follow up with a letter to EVERYONE INVOLVED! Your

former doctor, your current doctor, your health plan administrator, the CEO

of the company and if the results are the same, go to the media. You can

print out several studies that will show that most patient's ALT and enzyme

levels can return to 'normal' after beginning treatment! DUH.... the

interferon can help your liver function ... that's the reason it's a

treatment option for HCV!!!!!!!!!!! (Sorry, I just get so mad when I think

about the thousands of patients like you who are fighting an ignorant system!)

If you need assistance, write me and I can help!

> Question - should I go ahead and do my PCR labwork

> I'm PCR positive, then it's a moot point to

There is some speculation about the value of the PCR testing right after

treatment stops. Even if it is reduced or undetectable, the tell tale sign

will be the follow up testing... such as the first 6 months, the first year

and the very valuable test the second year. (However, I have seen patients

'relapse' after 3 years, but it's usually after trauma or a dramatic health

change regarding another health issue or immune system breakdown)

If you just want to see what your viral count is and if the PCR comes back

positive... DO it! (curiosity may get the best of you!)

But the chances of a 1A or 1B patient having total success after just over 5

months of combo is slim (not impossible, mind you so stay hopeful). However,

since you say you are feeling better since starting combo, then maybe this

was the trick to get your immune system to attack the virus in earnest.

Maybe you are slaying the dragon!

Dear alley,

I think you have a good point here. If you are

not negative now, then the chance of becoming negative

within one month is so very slim it would probably not

be worth the hassle of finding a new doc fast. I'm

sure you know the odds as well as I do. At least by

doing a PCR now it does give you one more piece of

info to work with a month earlier than you would have

had it. And if you are negative, that will be a very

good point to validate your position of wanting to

stay on treatment. I'd still be looking for another

doctor though.

No Alley.......This is your decision. I'm a normal enzyme hepper too...but

my diseae has progressed from the 1st -2nd biopsy I have moderate fibrosis

and a viral load of 5,000,000. Hepc just doesn't affect the liver! I think

it affects every part of my body....so that's an OLD thought he has.

GRRRRRR!!! I hear you!!!! gee

Sharon thanks for your post :) With your permission I will cut and paste it

to my PA who is a wonderful lady and doesn't agree with this gastro who will

forward it to the gastro.

I have had a biopsy - mild portal inflamation. Im working on getting copies

of labs.

My PA shows me my " chart " every time I see her and man, I gotta tell ya, if

nobody knew I was on treatment, you couldn't tell it much by my labwork.

Everything still pretty " normal " . I think the most change is in the thyroid

and that only by 0.X points up and it makes me hotter than most people lol.

But hey, I'm a hot woman! :)

I doubt he's trying to save money. Heck, I don't have an HMO, I'm allergic

to HMO's! :) And I've never met this guy. He told the PA when I asked to

speak to him on the fone cause I felt so bad this past week from a sinus

infection and headache, that he " doesn't talk to patients on the fone " .

Well, I had a lot of 4 letter words for her to relay to him!!!

Does the term " ANAL GASTRO " mean anything? haaaaaaaaaaaaaaa

OK.. I'm venting more.. I should calm down hehe but man... some people!

Well... if I am PCR zero at the 6th mo, they wil let me continue treatment.

If I am PCR positive at 6th mo, they will pull me off treatment. So I'm

wondering what a few weeks in my 5th month difference it would make.

Excellent post, Sharon. Time to be a bit proactive in continuing your tx,

Pat. I don't think it's wise to stop early. Your monthly viral counts have

been undetectable so far, right? The six month test just gives you a more

precise viral load count. It's sent to a lab that measures it to under 1000

parts per something <~~forgive me. I'm no nurse. I had that test in Aug.

If you quit now, it might all be for naught. That's my opinion.

You've come so far...

It's wearing, for sure. But we have our moments of pep, right? Not right

now...been staring at this screen...expect extreme dullness in your 8th(?)

Not right

now...been staring at this screen...expect extreme dullness in your 8th(?)

mo...hehe.

Peace,

Jane
