Hi Mandy

Well i started out being able to walk miles one week and the next week HBP rapid

heart beat and unable to walk miles any more. I was put on atenolol which helped

the sweating but I was still not right.A year later 05 my thyroid went hypo and

I requested Armour

and I slowly got worse in months to come, really higher bood pressure an below

range in K. I went through losing allot of hair due to the testosterone problem

I also have. I was at the end of my rope last year and this year. I had seen 4

Endos in the last year with no luck at all untill I found dr. Grim in here. He

seemed to know right off what I had. Labs confirmed Conns. My stomach has been

grumpy since 2001 and is worse now that it has ever been. I am sure it is all

adrenal hormones related as when I was on Hydrocortisone life become normal and

well I could do things again. I am doing a dexamethsone surpression test sunday

to see if I have GRA type like Dr. Grim thinks I could since I did so much

better on HC.Feel free to ask away.

How did you find out you had Conns? ita

Hi Mandy

I been sickly since 91 when I became sensitive to salt. I have told many dr.s

about my all of a sudden blood pressure and swelling since 04. I dont trust any

Endos in Sacramento as all 7 of them since 01 acted like I was some kind know

nothing like I was not aware of my own body going hay wire. Yes I gained 40lbs

not happy about that. I was even in ER with high blood pressure, very dizzy and

low potassium last year and they told me to eat banannas. When my potassium got

real low in may 06 I was so out of it I didnt even bother going to ER as like

before they did nothing. I didnt know it was low potassium again. I just started

taking hydrocortisone and got better, now I take spiro. Do you have a adrenal

tumor like most of us?

I do and figure one day it will have to be removed if I dont get better. I have

been real sick 2 years now and cant take it for much longer. If it were my thumb

doing this to me I would have chopped it off by now.

Do you have other out of wack hormones? ita

Just wanted to remind you all of my guide to difficult HTN which you should sent to all your old Drs who missed the problem so they can help the new pts coming to them.

Here it is and it is files at bloodpressureline in both .doc and .txt format. What happens when you try to access it?

Difficult Hypertension

CE Grim, BS, MS, MD, Hypertension Research, Medical College of Wisconsin

414-374-2733 lowerbp2@...

1. Review patient history; esp for low K problems.

2. PE for café-au-lait spots, fundi, heart, chest, upper abd for bruits.

3. Labs

* Lytes, urine albumin, Plasma aldosterone/plasma renin ratio-to screen for excess aldosterone or mineralocorticoid production.

4. Review BP device. If they don't have one, have them get an Omron or AND device with right sized cuff (arm circum >33 use large cuff).

5. Instruct on self BP measurement: sit 5 min, take 3 write them all down and average last 2.

* Take BP in am before taking Rx and then any other time they feel like BP is up or they are dizzy.

* Record in book and bring in.

6. Have the pt get the DASH Diet for Hypertension Book by , read it and use it for the 14 day test. They may wish to go to bloodpressureline for support. I run this site.

7. Review medications

* If not on a diuretic ALWAYS start one-HCTZ 1/2 25 mg (costs $8/100) have them buy this.

* Consider stopping all other RX and placing on the following combination

* Lotrel 2.5/10 bid

* Bisoprolol 2.5 / HCZ 6.25 each am or bid.

* Have pt call with BPs in 2-3 days.

* If not at goal increase Lotrel two am( and two pm and BIS to two am, two pm.

* If BP not at goal in two weeks then add Minoxidil 5 mg each am. Increase every few days by using 5 mg am, 5 pm, 10 mg am, 10 pm etc. Pt needs to weigh daily. If weight goes up then add furosemide 40 bid and increase. If still edema add metolazone 10 each am.

8. If BP not at goal and Aldo/Renin ratio high then add Spironolactone, 50 mg/day and may increase to 300 mg per day.

9. If the patient fails this combo do:

* 24 hr urine for catecholamines , Na, K and creatinine.

* Review diet for sodium should be less than 80 mM Na per day about 1500 mg.

* Consider regular renal arteriogram-not MRA or Nuclear scan, the only way to exclude renal artery stenosis as a cause of HTN is by selective transfemoral angiography to get good details of main and branch renal arteries.

RAPID BP CONTROL IN THE HOSPITAL

1. Take BP every hour.

2. Give HCTZ 25 po q 12 hours. If edema or Cr >2 use furosemide 40 q 12.

DRUG RESISTANT HTN, A SPECIALIST'S APPROACH: VD Alla, SJ Murtaza, C E Grim*, Medial College of Wisconsin, Milwaukee, WI. Presented at Am. Soc. HTN, 2000.

Many hypertensive patients who fail to achieve goal blood pressure (BP) or have side effects are referred to special clinics for management. In our experience many will need 4 or more BP meds for control. In the last 3 years we have evolved an effective approach to control BP in this outpatient setting. The near simultaneous release of two new long acting drug combinations, Ziac, which contains the long acting beta blocker bisoprolol (2.5, 5 and 10 mg) combined with 6.25 mg of hydrochlorothiazide (HCTZ) and Lotrel which contains the calcium channel blocker amlodipine and the ACEI benazapril (2.5/10, 5/10 and 5/20) suggested a useful way to approach these difficult patients. This "LoZi" cocktail combined the administration of 4 classes of drugs in only two pills once a day with the ability to start at a low dose and titrate up. HCTZ was usually added at 25 mg/d. LoZi produces increasing blockade of the 4 major BP control systems: vasoconstriction, RAAS, SNS and sodium retention.

We used this cocktail in 21 referred patients who had a BP of 183±24(SD)/108±12 on their referral medications at first consultation. At the second visit (2-4 weeks) the BP had decreased to 152±7/90±10. The combination has been effective in patients with severe HTN due to renal artery stenosis, mineralocorticoid excess and in HTN with resistant CHF.

In 10% of the pts the ACEI was stopped because of cough and in 5% the Ziac because of BB side effects. Our impression is that LoZi is a simple and effective combination to improve BP control in this group of patients. Controlled trials comparing this "combination of combinations " approach to others will be of interest in patients with "drug resistant" hypertension.

When did you find out you had conns and go on spiro? I found out in June of

this year when I found this news group.

Its amazing how dr.

grim knew what is wrong

with me after seeing so many dr.s in the last year and feeling so desperite as I

knew I was not well and had a almost knock down fight with one Endo as she dont

know what she is doing and the other one in the same office is no better.

I will take my chances without them as they were on their way to kill me and I

cannot trust their judgement at all no more.

I do have a very good family nurse practioner who has done more than the 4 Endos

i seen in the last year, like put me on spiro and did the labs to prove I have

Conns. This has been the most scary horrible place in my life and I cant wait to

get well some day one way or another. I am also hypothyroid and cant take enough

thyroid meds to be well thyroid wise as when I do it about kills me and I start

really releasing hormones from my tumor. I am so sick and tired of being sick

and tired.

It take time to recover from a low potassium experience weeks for me. Are you in

the U.S.

I am in Calif 1 hour from sacramento.

I got msn on my main computer but I kick back in bed on pilows on this reciever

called webtv and now its called MSNtv. It makes accessing newsgroups so easy

compared to the real computer.

I can run them both for 19.95 a month. ita

Thank you. If I try to access it from the PA site by

clicking on the files within the folder I get a " page

cannot be found " error for each link. I'm the one

that got the $27,000 bill for AVS from U of M and it

was no error, that's what they billed to my insurance

company. Fortunately the hospital accepted my

insurance so they had to accept what Blue Cross paid

them, which if I remember right was about $1,600. The

only bill I received was for $300 which was my portion

of the physician's fee for doing the AVS. The U of M

drs. don't accept my insurance but the hospital does.

Seems sort of strange since the two doctors I saw were

hospital employees and not in private practice. And

it still blows me away to think that if I did not have

health insurance I would be liable for the entire

$27,000 bill!

That is odd with your computer something is not on to allow you to open such

stuff. If you call your ISP or computer manifacture they might be able to walk

you through it. Any way in the mean time I can fax you the Evolution of

Diagnostic Criteria for Primary aldosteronism written by dr. Grim if you want,

write me at meling333@...

ita,

Thanks for offering but I can open the Evolution of

PA paper just fine. It's the files in the folder

labeled Difficult Hypertension that I can't open and

Dr. Grim posted them.

>I checked with our Radiology billing office and was told this

must have been due to a billing person going crazy and billing for

things they should not have billed for--for exampel the right side and

then separtely for the left side. and for renal veins as well as

adrenal. I would suggest you request an investigation of this

billing by your local medical board and by your insurance company.

That is odd with your computer something is not on to allow you to open

such stuff. If you call your ISP or computer manifacture they might be

able to walk you through it. Any way in the mean time I can fax you the

_The beauty of Conn' s is that if you have only 1 sided

disase then all of your Sx will likley be cured. You story is on

I have been hearing for 40 years. One of the reasons that I like HTN is

that we frequently find a cure for those that have been suffering for

years. Not too many diseases you can say this about.

When did you find out you had conns and go on spiro? I found out in June of this year when I found this news group.

Its amazing how dr.

grim knew what is wrong

with me after seeing so many dr.s in the last year and feeling so desperite as I

knew I was not well and had a almost knock down fight with one Endo as

she dont know what she is doing and the other one in the same office is

no better.

I will take my chances without them as they were on their way to kill me and I cannot trust their judgement at all no more.

I do have a very good family nurse practioner who has done more than

the 4 Endos i seen in the last year, like put me on spiro and did the

labs to prove I have Conns. This has been the most scary horrible place

in my life and I cant wait to get well some day one way or another. I

am also hypothyroid and cant take enough thyroid meds to be well

thyroid wise as when I do it about kills me and I start really

releasing hormones from my tumor. I am so sick and tired of being sick

and tired.

It take time to recover from a low potassium experience weeks for me. Are you in the U.S.

I am in Calif 1 hour from sacramento.

I got msn on my main computer but I kick back in bed on pilows on this

reciever called webtv and now its called MSNtv. It makes accessing

newsgroups so easy compared to the real computer.

I can run them both for 19.95 a month. ita

Dr. Grim is lowerbloodpressure ID in here he is a expert on Conns and has Conns himself. He created this news group for people line us. It is a shame you were not put on spiro last year. I was at a point where I would not go out alone either. Too risky as I was feeling like I would fall over or pass out.

Now I am better but still sick.

Have no energy or motivation.

I only do what is necessary and lay around like a beached whale.

Well actually I don't have Conn's myself but have cared for perhaps 100 patients with this. My BP is excellent and glucose and cholesterol are excellent but I struggle with my weight all the time.

May your pressure be low!

Sorry Dr. Grim I dont know where I got the idea you had Conns 10 years ago. Must

have been brain fog before I went on spiro.
